my OSHC Terms & Conditions
I confirm that as the Parent/Legal Guardian of the child named within this enrolment, have read, understood, and agree to abide by the following terms and conditions:
- The information given by me in this enrolment is true and accurate to the best of my knowledge. I have made full disclosure of all relevant information necessary to enable the Service to fulfil its duty of care to my child and others at the Service. I shall update this enrolment promptly of any changes to the information provided by me or any accompanying Medical Action Plans, Court Orders, and so forth, which may have a bearing on the health, welfare, care and safety of my child or any other persons.
- I understand if my child has a Medical Condition indicated on this form that they CAN NOT attend the service until I have signed and supplied all necessary documents and medication.
- I understand that this form is a Compliant Written Arrangement between myself as the legal guardian and my OSHC, which is required for Centrelink purposes.
- I understand of the opening and closing hours for the service. I shall adhere to the collection of my child by myself or another authorised adult, prior to closing time. I accept and understand the Late Pick-Up Procedure.
- I understand the service’s payment method is via Direct Debit.
- I agree to pay all fees associated with the Direct debit system.
- I understand if my payment is declined 3 times in a calendar year then a $200 bond is applicable to continue to use the Service.
- I understand it is my responsibility to ensure that there are sufficient clear funds available in my account to allow a debit/credit payment to be made in accordance with the Direct Debit Policy.
- Where parents/guardians cannot be contacted, I understand my OSHC staff as agent for the Parent/s, shall have the discretionary power to provide such medical attention as deemed necessary. I will pay all medical costs incurred on behalf of my child.
- I consent for OSHC staff to exchange information relating to my child with school staff and appropriate persons (i.e. an emergency situation – special needs for my child). I understand that this information will be handled confidentially.
- I release the Service from any liability to my child or myself in relation to any injury or illness that my child may suffer, and for loss or damage to property, in connection with the Service's activities, except to the extent that the liability arises as a result of the negligence of the Service.
-I understand that by booking my child into a session I am giving authority for my child to participate in the day’s programmed activities.
- I consent for staff to provide my child with basic first aid.
- I understand that in the interest of child safety it is a condition of entry to the service that all persons MUST provide photo identification upon request, my OSHC will make a note of this identification to keep on file.
- I understand that my OSHC may take photos and/or videos of my child during program activities and events.
- I consent for my child to watch G and PG rated movies.
- I consent for OSHC staff to apply sun block to my child when required.
- I understand my child cannot play outside without a bucket hat if the sun rating is over 3. I will provide a bucket hat for my child or my OSHC staff MAY provide a logo hat with the cost added to my account.
- I understand all sessions booked are reserved for my child and consequently will be charged for.
- I understand the my OSHC Cancellation and Absence Policy:
Cancellations - FREE OF CHARGE:
- HOME BASE DAYS: Bookings cancelled outside of 7 days from the day of booking will be cancelled free of charge.
- EXCURSION / INCURSION DAYS: Bookings cancelled outside of 14 days of the booking will be cancelled free of charge
Absence - FULL FEE (Minus CCS):
- HOME BASE DAYS: Bookings cancelled within 7 days of the booking will be classed as an Absence Day.
- EXCURSION / INCURSION DAYS: Bookings cancelled within 14 days of the booking will be classed as an Absence Day.
UNLESS:
The service is contacted by 8am the morning of the session and is provided with a Medical Certificate by close of business that day.
OR
The service is notified 48 hours or more before the day of the booking and a letter/ copy of text message or copy of email from the employer that states the parent is no longer required to work. The letter needs to be supplied before close of business that day.
OR
The service is notified of cancellation and a child on the waiting list can fill the booking.
- I understand that by booking my child into excursion days I am giving permission for my child to attend that excursion and I understand they will be travelling by bus unless otherwise indicated.
- I understand that during EXTREME weather my OSHC reserves the right to change the advertised program to a more suitable arrangement.
- I understand that my child must bring recess, lunch and water bottle daily along with enough snacks for whole day.
- I agree to purchase a my OSHC logo bucket hat for use on excursion days and understand my child will not be allowed to participate in the excursion without this.
- I understand Risk assessments have been conducted on all incursions / excursions and are available upon request.
- I agree to all of my OSHC policies, which are available on request, including but not limited to, Enrolment and Orientation policy, Behaviour Management policy and Fees policy.
